*Terms and Conditions of Contest: You must be eighteen years of age or older to have surgery. Surgery will be performed by Dr. Robert Selkin M.D and does not include follow up visits. Surgical enhancements excluded. Drawings will be held monthly. Drawing is for correction in one eye only. Free procedure is traditional LASIK only. Specialized LASIK or PRK is available, at an additional charge. Winner will be notified via telephone. Procedure must be performed within three months of the drawing. Patient must be determined a candidate for LASIK by Dr. Robert Selkin. We reserve the right to use each contest winner's name and photo for promotional use. Multiple entries will be discarded.
